Hospice Hawai‘i expands at-home care program

Hospice Hawaii has launched a new patient outreach program while expanding its medical team, adding geriatricians Diane Faran and Lam Nguyen to offer more at-home health services to terminally ill patients and their families.

Hospice’s new outreach program will involve physicians making more patient home visits to deliver end-of-life care rather than housing patients in the traditional facilities, such as those Hospice Hawaii operates in Kailua and Palolo on Oahu and at service centers at Queen's Medical Center and Molokai General Hospital. The nonprofit offers end-of-life and palliative health care to an average of 60 patients and their families every year.
