Residents who desire a state identification card may apply for one from 8:30 a.m. to 2 p.m. Sept. 19 at the Mililani High School cafeteria, 95-1200 Meheula Parkway in Mililani. The fee, $10 for senior citizens and $15 for all others, must be paid in cash.
The applications will be processed and the cards mailed to applicants within 10 business days. New applicants must provide an original Social Security card and a certified copy of their birth certificate or resident alien card.
For renewals with no change in name or citizenship, no other documents are required. Status or name changes due to marriage, divorce, annulment, adoption or citizenship must be supported by legible, unaltered, certified, government-issued documents.
